When you vault over an object and crouch at the same time you get stuck in "crouch mode" and have to manually tap to get out of it.
Its most commen when crouch jumping around.
i have mine Crouch/slide set to hold.

Good find, But you should add a video of you doing it, So it's much easier for the devs to find it

I probably can make an hour long video of bug spots, like the supply box exploit or the car flipping glitch and alot more.
But i can't be bothered.
